Do you have a Claim Number?
For consumers where we have a record of your purchase, we have emailed you a Claim Number at the email address we have on record around January 18, 2018. The email and Claim Number would look something like the image below.
You can still file a claim if you don’t have a claim number, just use this form.
If you don’t want to file a claim online, you can also print a claim form and mail it back to us.
